Health Insurance

All registered students are required to have health insurance coverage. Students who are not covered under a private plan must purchase the University Student Health Insurance Plan (U-SHIP).

Students who require a letter certifying U-SHIP coverage should contact the United Healthcare Insurance Coordinator at 773-834-4543. Students who require certification of enrollment at the University of Chicago should contact the National Student Clearinghouse.

Immunization

All registered students are required to show proof of immunization against certain diseases. The immunization requirement is outlined in the Immunization section of the Student Manual of University Policies.

Questions regarding immunizations or immunization records should be directed to the Immunization Office at 773-702-9975. The Immunization Office is part of the Student Care Center.

Note: Retain photocopies of materials submitted to the immunization office.
