Missing Information on the Transcript
AP/PT/IB Credit
First-year students: AP/PT/IB credit is posted to the transcript during the first year of study.
Second-year students: If AP/PT/IB or other credit is missing from the transcript, contact your College adviser. Official AP score reports are available from the College Board and should be sent to the University of Chicago, college code 1832.
Third- and fourth-year student transcripts should be up-to-date. If they are not, contact your College adviser. The deadline to submit missing documentation is the end of the quarter preceding graduation.
PE or Language Competency notation
Contact Stephanie Latkovski if the notation "College Language Requirement Completed" is missing from the Accreditation section of the transcript.
Contact Lorraine Galvin if the notation "Physical Education Requirement Completed" is missing from the Accreditation section of the transcript.
Grades
Contact the instructor or TA about missing grades. If the grade has already been submitted, contact Sharon Hudak in the Office of the University Registrar at 773-702-7891 to confirm that the grade was received. Contact your College adviser if the situation remains unresolved.
Late grades should be submitted by the instructor on department letterhead, and should include the quarter, class name, number, and section number, along with the student name and ID number. The registrar cannot accept grades sent by e-mail or delivered by students.
Transfer Credit
If the transfer credit petition was approved but the appropriate notation is missing under Transcript Notations: Previous Institutions Attended section of the transcript, contact your College adviser.
